A-State International Programs Pre-Arrival Attendance Form

For all students arriving to A-State, this form is required to be completed after you receive your visa and prior to your arrival to A-State. Each question is required for you to fill out. This should take you about five (5) minutes to fill out.
We are excited to welcome you for Spring 2026! Orientation date is on January 5th and this form will close on December 19th. There will be NO late arrivals. If you cannot complete this form by December 1st, please defer to a future semester.

Document Upload Section--Immigration Documents

Before you are able to submit this form, you are required to upload some documents. There will be three documents you are required to upload: 1.) a copy of your immigration form (either an I-20 or DS-2019), with your signature on it; 2.) a copy of your passport biographical page; and 3.) a copy of your F-1 or J-1 visa page. You will need to upload them in either a PDF format or a picture inside of a Word document.

Document Upload Section--Immunization and Flight Documents

This section is optional. If you do not have any immunizations from your home country, please skip this step. You will get them when you arrive. However, if you have an immunization booklet, please send them in for review prior to your departure. What we require is two doses of the Measles, Mumps, and Rubella immunization. The immunization information you submit should be in English, display your name, include the name of the immunization(s) and dates taken, and be signed by a doctor. If your immunizations are not in English, you may email us to get a copy of a form that your doctor can fill out that will also be accepted. Please note, again, that if you do not have these immunizations/tests, it is okay. You can get them upon arrival. Regardless if you do or do not submit your MMR immunizations, you will be required to take a blood test for Tuberculosis upon arrival.
